  • CSC Members must present their original physical or digital membership card (via CSC app) at the time of appointment to enjoy membership rate. Photocopies or screenshots are not accepted.
  • Services are available to dependants when accompanied by the member during the appointment. The member must present their membership card on behalf of the dependant. 
  • Member rates apply to listed treatments only. All other dental treatments are charged at prevailing rates. 
  • Walk-ins are not accepted. For same-day scaling appointment, SMS 8511 2025 with "Scaling". 
  • Not valid in conjunction with any other government subsidies, discounts, offers, privileges, or membership/loyalty programmes unless otherwise stated.
  • Khoo Teck Puat Hospital reserves the right to amend these terms and conditions without prior notice.
  • Collection of personal data by Merchant for this promotion is strictly for their own business requirements. CSC is not responsible for the collection, use and disclosure of the Personal Data by Merchant for this promotion.
